Our Story

Peoples-Parker Solutions Group delivers the best care possible in order to let the individuals we serve understand they are valued members of our communities. Every individual that we work with receives uniquely personalized care to improve their lives and encourage positive outcomes from their treatments.

Support Group Session

RECOVERY MANAGEMENT

We are a Texas based Recovery Management entity that provides home and community-based services to adults with serious mental illnesses.

We understand the best ways to leverage appropriate community resources to get our clients what they need, when they need it.

Our Recovery Managers (RM) coordinate, link, advocate, monitor, and make referrals for each of our clients.

RMs work with clients to ensure their treatment is individualized and unique to their needs.

PROMOTING PERSONAL GROWTH

We recognize that the individuals we serve have moved between many institutional settings like jails, hospitals, emergency rooms, and long-term psychiatric facilities. That’s why we wish to promote their recovery with a comprehensive array of services to allow for successful reintegration into their chosen communities. The Person Centered Recovery Plan (PCRP) is the collaborative process utilized by the Recovery Manager (RM) and directed by the individual, to create the Individual Recovery Plan (IRP). PCRP uses a recovery orientation and team approach that includes the individual, formal supports, and natural supports to best support the individual’s recovery and goals and support to ensure that the individual directs the process to the maximum extent possible, and is enabled to make informed choices and decisions.

REFERRAL & ENROLLMENT

HCBS-AMH focuses on three targeted populations.

Applicants must meet only one of the following targeted populations.

 

MUST HAVE:

  • Serious Mental Illness diagnosis

  • Medicaid (Medicaid Eligibility)

  • 18 years of age or older 

and fulfill 1 of 3 of the following:

  • 4 Jail arrests and 2 crisis stays within the last 3 years

  • 15 emergency department visits (any nature) and 2 crisis stays within the last 3 years

  • 1,095 days in-hospital over a 3–5-year period.
